Obtaining birth certificate in Uttar Pradesh

 

Every child birth that takes place in Uttar Pradesh has to be registered within days and after one year first class magistrate order would be needed for registration of births. Wherever the birth has taken place, concerned in-charge person has to report the matter to the local authorities.

Birth certificate in Uttar Pradesh can be obtained from the local body authorities.  For this an application in the prescribed proforma has to be submitted and the local bodies in the zone in which the parents and the child are living would issue the birth certificate. Authorities that are concerned for issuing birth certificates in Uttar Pradesh are the Municipal Corporation, Nagar Palika and Nagar Palika Parishad.

For obtaining birth certificates in Uttar Pradesh including major cities such as Lucknow, Kanpur and Jhansi, application form along with necessary fee, proof of birth of the child or of the person who requires birth certificate have to be submitted.  If the birth is being registered after one year, affidavit is also required
