Which form to be used for applying for voter ID card? | Address of Indian Electoral office, Officer of main election and Government gazette officer
If you are newly applying for voter ID card, use Forum -6. Those who have got 18 years of age as on 1st January 2011 are eligible to get voter ID cards. Attach two passport size photos. Certificates which give the details of age are to be attached. For instance, birth certificate orÂ school / college certificate or affidavit from parents are to be submitted. Apply before 5th December 2010 for registration.
