Births and Deaths department of Meghalaya deals with the registration births and deaths. Every death in Meghalaya has to be informed to the concerned municipal board within 21 days from the date of occurrence of death. For obtaining death certificate the applicant can download the application from the website of they can personally collect the application form in SMB office in Birth & Death department. The death certificate will be issued within 24 hours from submitting the application.

In case of delayed registration of deaths in Shillong, Baghmara, Cheerapunjee in Meghalaya the death can be reported within thirty days by paying late fee prescribed. For registering deaths within one year of from the date of occurrence, written permission of prescribed authority and an affidavit made before the notary are required. The order of the Magistrate of first class or Presidency Magistrate of Meghalaya is required to register deaths after one year along with prescribed fee. Senior Medical & Health officer, Registrar of Births & Deaths is the concerned authority for issuing death certificates.

Documents that needed to be submitted to obtain death certificate in Meghalaya are the death certificate from locality headman if the death occurred at home, death certificate from the hospital if death occurred in the hospital.
