Concerned department for obtaining death certificate in Punjab is the Executive Office, in urban areas and in rural areas, the office of Civil Surgeon acts a District Registrar (Birth and Death). Any family member of the family of deceased can apply for death certificate.

For obtaining death certificate in Amritsar, Jalandhar, Patiala in Punjab, application forms can be downloaded from the website or can be obtained for free of cot from the Office of the Registrar (Births & Deaths).   The application form has to be filled and signed by the applicant and it shall be submitted to the concerned authorities.   The computer operator/clerk or cashier at the counter who receives the application gives a receipt along with the prescribed fee. The death certificate can be either received personally or by post if a self addressed envelope is given along with the application.
