Eligibility for applying to include names in electoral rolls in Punjab is –

  • The applicant shall be an Indian citizen.
  • He/she shall have attained the age of 18 years.

Following forms are to be used for applying to inclusion of name in electoral rolls in Amritsar, Jalandhar, Patiala in Punjab are Form 6 to include the name after the draft rolls are published.  This form need to be submitted to the Electoral Registration Officer.  Form 7 shall be used to delete the name from electoral rolls.  For modification of entries in electoral rolls, Form 8 has to be used.

Documents that shall be attached for applying to register name in electoral rolls are one passport size color photograph, Date of birth certificate (Birth certificate issued by municipal authorities/birth certificate from the school/Tenth class mark sheet/Certificate of sarpanch), residence proof certificate (Bank/kisan/post office pass book/ration card/passport/driving license/income tax return/water or telephone or electricity or gas bill).
